Centro Radioncológico del Caribe treated dozens of patients daily with a registration process entirely on paper. Clinical records were filed in physical folders ordered by date: finding a patient's history could take up to 20 minutes. The administrative team duplicated information in spreadsheets, and doctors waited before each consultation while someone located the correct folder.
Furthermore, the radiotherapy equipment — high-cost and highly critical machines — lacked a centralized maintenance tracking system. Calibration and failure records were noted in notebooks. An undetected out-of-service machine directly affects the continuity of patients' oncology treatments.
